  • A Kaupapa Maori Needs Assessment provided that is culturally sensitive and appropriate to the needs of Kaumatua kuia and whanau.
  • A home visit assessment completed in their home environment or a venue of their choosing can be negotiated.
  • A holistic assessment model is used to capture the persons overall needs within their Taha Tinana, Taha Hinengaro, Taha Wairua, Taha Whanau.
  • A Care Plan will be developed along with Kaumatua Kuia and whanau
  • Kaumatua Kuia and whanau information will be protected
  • The Kaumatua Kuia Rights and Obligations will be provided
  • Providing education and awareness to Kaumatua Kuia and whanau of relevant information pertaining to their needs and supports readily accessible and available.
  • Assessor will monitor and follow-up care plan advocacy also provided.

  • Must be 54+ years of age
  • Living within Counties Manukau District Health regions
  • High complex needs
