A Kaupapa Maori Needs Assessment provided that is culturally sensitive and appropriate to the needs of Kaumatua kuia and whanau.
A home visit assessment completed in their home environment or a venue of their choosing can be negotiated.
A holistic assessment model is used to capture the persons overall needs within their Taha Tinana, Taha Hinengaro, Taha Wairua, Taha Whanau.
A Care Plan will be developed along with Kaumatua Kuia and whanau
Kaumatua Kuia and whanau information will be protected
The Kaumatua Kuia Rights and Obligations will be provided
Providing education and awareness to Kaumatua Kuia and whanau of relevant information pertaining to their needs and supports readily accessible and available.
Assessor will monitor and follow-up care plan advocacy also provided.

Living within Counties Manukau District Health regions
